FINAL WARNING – Third Missed Appointment
According to our records, you booked an appointment with your GP / Practice Nurse / Pharmacist / Healthcare Assistant and then did not attend, with no reason given. This is now the third instance of non-attendance to an appointment that you have booked without a reason advised to us.

We understand that on occasions there may be genuine reasons why you may be unable to attend an appointment. We respectfully ask you to contact us via our online services, text message, email, telephone or in-person, to let us know when this has occurred at least an hour in advance, so your appointment can be offered to someone else.

Wasted appointments prevent other patients who want to see a clinician in our team unable to do so due to limited availability.

Since this is now the third instance of non-attendance to an appointment that you have booked, we need you to please contact us within 5 working days of receiving this notice, stating the reason for your non-attendance on this occasion.

We have approximately 200 unattended appointments per month. We deal with non-attendance and lateness proactively so that our services are not strained, and to offer the best possible access to all of our patients.

We normally send two notifications for missed or late appointments before further action is considered, i.e. on the first missed appointment and on a second missed appointment.

Due to this being a third instance of non-attendance, we are now in the process of removing your registration from the GP surgery, unless a valid reason is given within 5 working days.
